In consideration of being permitted to take an optional electric-vehicle ride-along at the Otonamo grand opening (Otonamo LLC, 1649 E McKinley Ave, Mishawaka, IN), I acknowledge that riding in a high-performance electric vehicle, including features such as rapid acceleration and driver-assistance or Full Self-Driving (Supervised) technology, involves inherent risks including collision, injury, death, or property damage, and I knowingly and voluntarily assume all such risks. To the fullest extent permitted by Indiana law, I release, waive, and discharge Otonamo LLC and its owners, employees, agents, and volunteers (the "Released Parties") from any and all claims for injury, death, or property damage arising from my participation, whether caused by the ordinary negligence of the Released Parties or otherwise; this release does not apply to gross negligence or willful or wanton misconduct. I agree to indemnify and hold the Released Parties harmless, to follow all staff instructions, and to wear a seatbelt at all times. I grant Otonamo permission to use photos or video of my participation for marketing unless I notify staff otherwise in writing. Ride-alongs carry up to three riders per trip. Each adult rider must sign their own release. By signing, I am signing for myself and, if applicable, as the parent or legal guardian of the minor children I have listed, and any other adults riding with me will sign their own release. This is a summary of the full release; a signature here is a legal electronic signature.
